At Sanus Medical, Dr. Paulk has been approved as a USCIS Civil Surgeon since 2015.
Form I-693, Report of Medical Examination and Vaccination Record, is used by the U.S. Citizenship and Immigration Services (USCIS) to determine if an applicant is inadmissible to the United States on public health-related grounds. This form may be necessary if you are applying for adjustment status to become a lawful permanent resident and for asylum or refugee cases.

Our immigration manager is extremely detailed oriented, friendly, and understands the requirements that are necessary for applicants regardless of age.
We can usually schedule an exam within a couple of days of initial contact, assuming proof of vaccine information can be provided up front. We have an onsite lab, xray equipment and medical assistants willing to help you through the process.
